Form for State Supplemental Payment (SSP 22)

The SSP 22 is the form that must be completed in order to ensure that a youth in foster care who has been found eligible to receive SSI/SSP benefits receives the maximum amount of SSI/SSP available. Youth in foster care qualify for the "Non-Medical Out of Home Care Rate" which means they can get up to $1,042 a month in SSI/SSP benefits - but only if the SSP 22 is completed!
